Payment Information
CPHP is a Fee-for-Service practice meaning I do not accept insurance as a form of payment for services. Instead, the patient is responsible for payment at the time of service. I accept cash, checks, and credit cards (Visa, MasterCard, American Express) as forms of payment. If you pay by check, you should know that there will be an additional $40.00 fee for checks returned for insufficient funds.
